 4.  Submission of Application form to the respective institute:

The candidates seeking admission to the first year of Diploma Courses and eligible to apply for the same, shall submit separate application forms complete in all respect to the institutes of their interest.

4.1  Application form Receipt centres:

Note : Candidates seeking admission against seats reserved for N.C.C. candidates can also submit Separate application form as Type-A ,B.C, D or E candidates as the case may be to the individual institutes.
3 Jammu and Kashmir Migrants. The Principal, S.B.M. Polytechnic, Near Cooper Hospital, Vile Parle (West), Mumbai 400 056.
4 Outside Maharashtra State (OMS) Candidates. Principal of the Respective Un-Aided Polytechnics
5 Foreign Nationals and Persons of Indian Origin etc. The Principal / Head of the Dept. of respective Govt. , Non Govt. Aided and Un - Aided Polytechnic where the candidate is seeking admission.
6 Marathi speaking candidates of Mauritius (MR) The Director of Technical Education, Maharashtra State, 3, Mahapalika Marg, Dhobi Talao, Mumbai 400 001
4.2  Documents required for various types of candidates:

The documents required to be produced by various types of candidates at the time of admission are as shown in Annexure-C. The candidates have to attach the attested copies of the documents mentioned in Annexure-C along with their application form.

4.2.1  Attestation. The candidates shall note the following points while submitting the Application forms:
  • The candidates shall attach attested copies of all the required certificates as mentioned in these rules and enumerate the same in the given format. The candidates shall not attach a copy of any other certificate which is not asked for under these rules such as certificates for participation in sports, cultural activities etc.

  • The copies of certificates or documents attached to the application form should be attested by the Principal of the College from which the candidate has passed qualifying examination or Gazetted Officer or Special Executive Magistrate or Head Master of a Secondary School or teaching staff of a Government college / Polytechnic not below the rank of a Lecturer. Original certificates should not be attached with the application.

ANNEXURE -A
Attested copies of the documents to be attached with application form:

2 Type-B Candidates Domicile certificate of father/mother indicating that they are domiciled in the state of Maharashtra.
In case of Mother's domicile certificate, the candidate shall submit an affidavit on Rs.10 stamp paper, stating that the person in whose name the domicile certificate is issued is the mother of the candidate.

3 Type-C Candidates 1. Transfer order stating that father/mother of the candidate is presently posted in Maharashtra.
2. Copy of Joining report after the transfer.
3. Copy of the certificate from the office of the father/mother stating that he/she is not transferred outside Maharashtra after the transfer mentioned in 1. Above (In addition to the documents mentioned in Sr. No. 1)
4 Type-D Candidates and
Type-E Candidates
Certificate from the employer stating that father/mother of the candidate who is a Maharashtra State government/ Maharashtra state government undertaking employee is presently posted in Maharashtra after completing deputation outside Maharashtra / Posted in the present district after being transferred from another district.

5 Backward class candidates belonging to S.C. / S.T / V.J. / D.T / N.T. 1 / SBC categories. Caste certificate issued by Executive Magistrate / Sub Divisional Officer

6 Backward class candidates belonging to N.T.-2 / N.T.-3 / O.B.C. categories 1. Caste certificate issued by Executive Magistrate/ Sub Divisional Officer.
2. Non creamy layer certificate issued by the competent authority issued after 1st April of last Financial year.

7 Defence Service Personnel Certificate in 'Performa' in application form the appropriate authority, certifying that the candidate fulfils the conditions as per Rules 3.3.

8 Physically Handicapped Candidates Certificate in 'Proforma' in application form the Director All India Institute of Physically Handicapped, Mumbai or Civil Surgeon regarding candidate's physical disability, and ability to undergo all parts of syllabus such as theory, practical, workshop etc. for the course.

9 N.C.C. Candidate 1. Certification of representing Maharashtra at all India level.
2. Certificate of minimum 70% attendance in two years of N.C.C.
3. N.C.C. - certificate.

10 Jammu and Kashmir Migrant candidates Necessary Certificates in support of J and K migration.

2. Certificate / Proof of Foreign national or

3. Certificate of passing the qualifying examination from an institution outside India.
Domicile certificate in respect of his father /mother

4.2.2  Translation:

If the candidate produces any certificate, which is not in Marathi or Hindi or English language, authenticated Marathi or Hindi or English version of the same, duly attested by a Gazetted Officer shall also be produced.
